Use the following Check List to collect your tax information. It will help you remember all important information relevant to your tax preparation. The list is comprehensive, however you only have to pick what is applicable to you.

Personal Data

  1. Social Security Numbers and Date of Birth (including spouse and children)
  2. Child care provider : Name, address and tax I.D. or Social Security Number
  3. Alimony paid: Social Security Number
  4. If you are a new Client, please send –
    1. Your ID like SSN Card/ Driver’s License/ Passport Copy
    2. Your spouse’s ID like SSN Card/ Driver’s License/ Passport Copy
    3. Your Dependents Birth Certificate/Passport copy with I-94/School record/Immunization Certificate

  5. If you are Filing NY & AL State Taxes, (Resident, Non Resident, Part year resident) please send us a copy of your & your spouse’s Driver’s License/State Issued ID. These can be issued by any State
